From the start it's easy to note some major differences between Italian and English that contribute to difficulties in translation between the two. Italian is a romance language and features a lot of connections with Spanish and French. English, on the other hand, is a Germanic language. Though important, the differences between these two language families are often overstated and their common Indo-European roots are often unfairly brushed aside.
For example both Italian and English utilize the same language of twenty six letters. Other languages, such as French, deviate from the English alphabet through the addition of a significant number of unique accented letters. The difference between the English and the Italian alphabets is actually one of subtraction- the Italian alphabet doesn't natively utilized a handful of common English letters (x,w,k,y and j), which are only utilized in Italian when spelling words of explicit foreign origin. Otherwise the main differences in alphabet between the two languages simply relates to the differing pronunciations of a few letters, which makes Italian translation and interpretation occasionally tricky for novices.
One of the most interesting differences in pronunciations between Italian and English is the fact that words in the former language tend to finish with vowels. Because of this difference many Italian speakers who are learning English will unconsciously attach additional vowels to the ends of their English words, you may experience this when using an Italian interpreter. There are a number of other notable pronunciation differences between the two, such as the difficult of Italian speakers to handle "th" words, but overall it's this vowel addition that is most notable and prevalent.
Grammatically the two languages feature a few important differences. The most notable of these relates to the fact that Italian sentences are more flexible with the order of their words, as the meaning and associations between words tends to rely more on inflection than a fixed structure, making translation into Italian a simpler task. In general, however, most Italian speakers have greater difficulties mastering English pronunciation as the former is a far more phonetic language than the latter, resulting in significant difficulties when it comes to pronouncing or recognizing otherwise unfamiliar words.
It's far from impossible for English and Italian speakers to communicate with each other, but the significant differences between the two often results in a lot of misinterpretation.
